Job Description

An exciting opportunity awaits for an experienced and compassionate nurse specializing in home health. As a Registered Nurse providing care in the home setting, you will play a pivotal role in ensuring high-quality, patient-centered services. This contract position involves working independently in the field, making a significant impact on the lives of patients and their families.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel to patient homes with mileage reimbursement

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
  • Monitor and evaluate patient progress, communicate changes in condition promptly, and update care teams and physicians as needed
  • Educate patients and families on diagnosis, treatments, and preventive practices
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB)
  • Coordinate care with interdisciplinary team members including therapy and social work
  • Attend case conferences, staff meetings, and pursue continuing education as needed
  • Uphold all agency and regulatory standards

Qualifications:

  • Active, unencumbered RN license in the state of practice
  • At least 1 year of recent field-based Home Health RN experience
  • Proficiency with Home Care Home Base (HCHB) and solid OASIS experience required
  • Strong clinical assessment, critical thinking, and communication skills
  • Self-motivated, highly organized, able to work independently
  • Reliable transportation, valid drivers license, and current auto insurance

Preferred:

  • BSN degree
  • Experience managing high point caseloads (2532 points per week)
  • Familiarity with Medicare and home health compliance
