Seeking an experienced and compassionate professional to provide skilled nursing care to patients in the comfort of their homes. This field-based, full-time contract offers the chance to make a direct impact on patient outcomes while maintaining both clinical autonomy and a high standard of care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status, assess progress, and report changes to interdisciplinary teams and physicians.
• Provide patient and family education on treatment plans, disease management, and preventive health measures.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for coordinated care.
• Attend case conferences and staff meetings as necessary, supporting team-based care delivery.
• Ensure all patient care and documentation meets regulatory and agency standards.

Qualifications:

• Active RN license (in good standing).
• At least 1 year of recent Home Health experience, with direct fieldwork preferred.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS.
• Exceptional clinical assessment, decision-making, and organizational abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently while managing a diverse caseload (2532 points per week).
• BSN is preferred, as is prior experience with Medicare compliance and similar caseload volume.
