A rewarding opportunity is available for an experienced Registered Nurse specializing in Home Health. This role focuses on delivering skilled, patient-centered care in a home setting, promoting continuity and positive health outcomes for patients who need clinical expertise in the comfort of their own homes.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop individualized care plans.
• Deliver direct nursing care including wound care, medication administration, IV therapy, and management of chronic conditions.
• Monitor patient progress, report any changes in condition, and adjust care as necessary.
• Provide education and support to patients and families, ensuring they understand the care plan, medications, and preventive measures.
• Maintain up-to-date documentation in the Home Care Home Base (HCHB) electronic system.
• Work collaboratively with an interdisciplinary team including therapists, social workers, and home health aides to coordinate holistic care.
• Attend staff meetings, participate in case conferences, and engage in continuing education.
• Ensure compliance with all regulatory and professional standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• At least 1 year of recent Home Health field experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Effective critical thinking, assessment skills, and strong communication abilities.
• Reliable transportation, current drivers license, and insurance.
• Ability to manage a flexible, autonomous schedule and field-based caseload.
• BSN and experience managing 2532 points per week preferred.
• Knowledge of Medicare and home health compliance is a plus.

$2,490 - $2,634/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
