Make a significant impact on patients lives as a Home Health Registered Nurse, providing skilled, patient-centered care directly in clients homes. This contract opportunity is ideal for compassionate and experienced nurses who excel in independent, field-based roles and are committed to clinical excellence and collaborative care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Field-based with daily travel; mileage reimbursement provided
• Caseload: 2532 points per week (roughly 35 visits per day)

Key Responsibilities:

• Complete OASIS assessments and develop individualized care plans for each patient.
• Deliver comprehensive skilled nursing care, including wound care, IV therapy, medication administration, and management of chronic conditions.
• Monitor patient progress, recognize changes in condition, and communicate promptly with care teams and physicians.
• Provide education to patients and families about disease management, treatment, and preventive strategies.
• Document care and patient interactions promptly and accurately using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and aides to ensure coordinated, holistic care plans.
• Attend case conferences, staff meetings, and pursue ongoing education as required.
• Remain compliant with all agency, state, and federal documentation and care standards.

Required Qualifications:

• Active RN license in good standing in the state of practice.
• Minimum of 1 year recent home health experience in the field.
• Proficient in OASIS and Home Care Home Base (HCHB).
• Strong clinical, assessment, and time management skills.
• Reliable transportation, current drivers license, and auto insurance.
• Ability to work independently and maintain exceptional communication and organization.

Preferred:

• BSN degree.
• Experience managing 2532 points per week.
• Knowledge of Medicare and home health compliance.
