Experienced Home Health Registered Nurses are sought to deliver high-quality, patient-centered care to individuals in their homes. This contract position focuses on managing a diverse patient caseload and ensuring optimal outcomes through skilled nursing interventions. The environment supports autonomy, professional growth, and close collaboration with other healthcare professionals.
Schedule & Hours:
- Full-time, 13-week contract
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points weekly (typically 35 patient visits per day)
- Field-based role with daily travel (mileage reimbursement provided)
Essential Responsibilities:
- Complete comprehensive OASIS assessments and craft detailed, individualized care plans
- Deliver a wide range of skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management
- Evaluate patient progress, identify changes, and promptly communicate with physicians and interdisciplinary care teams
- Provide education for patients and families regarding disease processes, medications, treatments, and preventive care
- Maintain thorough, accurate electronic documentation through Home Care Home Base (HCHB)
- Participate in case conferences, staff meetings, and ongoing education
- Adhere to regulations and agency standards for care delivery and documentation
Qualifications:
- Active RN license in good standing
- At least 1 year of recent Home Health RN experience (field experience highly preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Reliable transportation, valid drivers license, and auto insurance
- Excellent clinical assessment, communication, and organizational skills
- BSN and experience managing 2532 points per week preferred
- Familiarity with Medicare and home health compliance standards is a plus
