An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in home health. This role focuses on delivering skilled nursing care directly to patients in their homes, ensuring individualized, high-quality, and patient-centered services. The position is suited for a confident RN who excels independently, demonstrates strong clinical skills, and values effective patient communication.
Schedule and Role Details:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (approximately 35 patient visits daily)
- Field-based: daily travel to patient residences (mileage reimbursement available)
- Documentation: Home Care Home Base (HCHB), OASIS assessment tool
Key Responsibilities:
- Perform detailed OASIS assessments and create tailored care plans
- Administer nursing care such as wound care, IV therapy, medication management, and chronic disease interventions
- Monitor and report patient progress, communicate changes in condition to physicians and care teams
- Educate patients and their families about care plans and health management
- Accurately document all care in Home Care Home Base (HCHB)
- Collaborate closely with interdisciplinary teams including physical/occupational therapists, social workers, and aides
- Attend case conferences, staff meetings, and participate in ongoing education
Qualifications:
- Active RN license (in good standing) in the state of practice
- At least one year of recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong assessment, critical thinking, and organizational skills
- Reliable transportation, valid driver's license, and auto insurance
- BSN and Medicare/home health compliance experience preferred
