Job Description

Take part in delivering exceptional patient care as a Home Health Registered Nurse, providing vital support and skilled nursing services to patients in the comfort of their own homes. This contract position emphasizes patient-centered care, continuity, and clinical excellence, allowing you to make a significant impact as you manage individualized plans and collaborate with interdisciplinary teams.

Schedule & Setting:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Field-based role with daily travel within the assigned territory (mileage reimbursement provided)
  • Caseload: 2532 points per week (typically 35 patient visits daily)
  • Utilize Home Care Home Base (HCHB) for documentation and complete OASIS assessments

What Youll Do:

  • Perform comprehensive OASIS assessments and develop individualized patient care plans
  • Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
  • Track patient progress, identify changes in condition, and communicate promptly with care teams and physicians
  • Educate patients and families on health conditions, medications, and preventive care to foster patient independence
  • Maintain timely, accurate clinical documentation in HCHB
  • Coordinate with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless care
  • Attend case conferences, staff meetings, and ongoing education sessions
  • Adhere strictly to all regulatory and agency standards

Qualifications:

  • Active RN license in good standing within the state
  • At least 1 year of recent home health experience, with field background preferred
  • Proven proficiency with Home Care Home Base (HCHB) and OASIS
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work autonomously and manage a dynamic, field-based schedule
  • Excellent communication, organizational, and critical thinking skills
  • BSN preferred; knowledge of Medicare/home health compliance a plus
