An opportunity is available for a skilled Home Health Registered Nurse (RN) to provide comprehensive nursing care in patients homes throughout the Roanoke, VA area. This contract position requires a compassionate and detail-oriented nurse who excels at delivering individualized patient-centered care while working independently in a field-based setting.
Responsibilities include conducting thorough OASIS assessments, developing and managing personalized care plans, and performing skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. You will evaluate patient progress, communicate changes promptly with healthcare teams and physicians, and educate patients and their families on diseases, treatments, and preventive measures. Accurate documentation using Home Care Home Base (HCHB) is essential, as is collaborating with therapists, social workers, and home health aides to ensure seamless, coordinated care. Participation in case conferences, staff meetings, and ongoing education will support your professional growth.
Key Qualifications:
- Active RN license in good standing for the state of practice
- At least 1 year of recent home health nursing experience, preferably field-based
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
- Strong clinical assessment, critical thinking, communication, and organizational skills
- Reliable transportation with valid drivers license and auto insurance
- Ability to manage a flexible, independent field schedule
Preferred:
- BSN degree
- Experience managing a caseload of 2532 points weekly
- Knowledge of Medicare and home health compliance standards
Schedule & Details:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload: 35 patient visits daily (2532 points per week)
- Mileage reimbursement provided
