An opportunity is available for a skilled and compassionate Registered Nurse with field-based home health experience. Youll play a vital role in delivering quality, patient-centered nursing care directly to individuals in their homes under a full-time, 13-week contract. This position emphasizes autonomy, thorough clinical assessment, and close collaboration with an interdisciplinary care team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive OASIS assessments for each new patient.
• Develop and manage individualized care plans tailored to patient needs.
• Deliver skilled nursing care, including medication administration, IV therapy, wound care, and chronic disease management.
• Monitor patient progress, promptly communicate health changes, and coordinate with physicians and the care team.
• Educate patients and families on diagnosis, medications, and strategies for achieving care goals.
• Document all visits and clinical findings using Home Care Home Base (HCHB).
• Collaborate with other professionals, such as therapists and social workers, to ensure holistic patient care.
• Participate in case conferences, meetings, and required staff training.

Qualifications:

• Current and active RN license in good standing.
• Minimum of 1 year recent home health nursing experience; strong field experience highly valued.
• Proficient using Home Care Home Base (HCHB) and experienced with OASIS assessments.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong critical thinking, communication, and organizational skills.
• Ability to work independently and manage a field-based caseload (2532 points/week).
• BSN degree and prior experience with Medicare compliance and high patient point caseloads are a plus.

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
