A skilled and compassionate Home Health Registered Nurse is needed to provide patient-focused care in the home setting. This contract offers the chance to deliver high-quality nursing services that make a direct impact in patients daily lives. The ideal professional is self-directed, organized, and comfortable working autonomously while maintaining excellent communication and clinical standards.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor and document patients progress, addressing changes and collaborating promptly with physicians and care team members.
• Educate patients and families about disease management, treatment plans, prevention, and medication use.
• Complete timely, accurate EMR documentation using Home Care Home Base (HCHB).
• Coordinate with other healthcare team membersPT, OT, MSW, and home health aidesto ensure continuity of care.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Follow all agency and regulatory requirements for documentation and service delivery.

Qualifications:

• Active RN license in good standing (state of practice).
• At least 1 year of recent Home Health RN experience.
• Proficient in HCHB and OASIS documentation.
• Reliable transportation, valid drivers license, and proof of auto insurance.
• Strong clinical judgment, assessment, and time management skills.
• Ability to thrive independently in a field-based setting.

Preferred:

• BSN degree.
• Experience managing 2532 points per week.
• Familiarity with Medicare and home health compliance.
