Provide skilled, patient-centered nursing care as a Home Health Registered Nurse (RN) in a dynamic field-based setting within the Ringgold, GA area. This full-time, 13-week contract role offers the opportunity to deliver comprehensive home health services while working independently and collaboratively with an interdisciplinary healthcare team.
Responsibilities include conducting detailed OASIS assessments, creating and managing individualized care plans, and delivering skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. You will evaluate patient progress, communicate changes promptly to the care team and physicians, and educate patients and their families on treatment plans and preventive care. Accurate and timely documentation in Home Care Home Base (HCHB) is essential, along with collaboration with therapists, social workers, and home health aides to ensure seamless care coordination.
Qualifications and Desired Experience:
- Active RN license in good standing in the state of practice
- Minimum of 1 year recent home health experience, with field experience preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment, critical thinking, organizational, and communication skills
- Reliable transportation with valid drivers license and auto insurance
- Ability to manage a flexible, independent field-based schedule
- BSN preferred
- Experience managing caseloads of 2532 points per week preferred
- Knowledge of Medicare and home health compliance standards a plus
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload of approximately 2532 points (35 patient visits per day)
