A rewarding opportunity awaits an experienced and compassionate Registered Nurse skilled in home health care. In this field-based contract position, you will deliver patient-centered nursing care within clients homes, focusing on high-quality, continuous service and positive patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and develop individualized care plans for each patient.
• Deliver skilled nursing services such as wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patient progress, identify changes in condition, and promptly inform physicians and care team members.
• Educate patients and their families on treatment plans, medication management, disease processes, and preventive strategies.
• Document all visits and interventions using Home Care Home Base (HCHB).
• Collaborate closely with the interdisciplinary healthcare team including physical therapists, occupational therapists, and social workers.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with all agency protocols and regulatory guidelines.

Qualifications:

• Active RN license in good standing (state-specific).
• At least 1 year of recent home health nursing experience; field experience is preferred.
• Experience with OASIS and proficiency in Home Care Home Base (HCHB) required.
• Strong clinical assessment, critical thinking, and communication abilities.
• Ability to work autonomously, confidently manage a field-based schedule, and maintain accurate documentation.
• Reliable vehicle, valid drivers license, and auto insurance.

Preferred:

• BSN degree.
• Familiarity with managing 2532 points per week and knowledge of Medicare/home health compliance.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
