Job Description
Deliver essential, patient-centered care as an experienced Home Health Registered Nurse, providing skilled nursing services for individuals in their homes. This contract opportunity is ideal for nurses who are highly motivated, organized, and thrive working independently while maintaining clinical excellence and delivering superior patient communication.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based (patient homes)
- Caseload: 2532 points per week (35 patient visits per day)
Key Qualifications:
- Active RN license (in good standing) in the state of practice
- At least 1 year of recent Home Health experience (field experience preferred)
- Proficiency with electronic documentation systems, especially Home Care Home Base (HCHB)
- Strong background in OASIS assessments
- Reliable transportation, valid drivers license, and auto insurance
- Excellent clinical assessment, time management, and communication skills
- Ability to work independently, managing a flexible, field-based schedule
- BSN preferred
- Prior experience managing a caseload of 2532 points weekly and familiarity with Medicare/home health compliance are valuable assets
Primary Responsibilities:
- Conduct comprehensive OASIS assessments and create individualized patient care plans
- Deliver skilled interventions including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress and coordinate care with interdisciplinary home health teams
- Educate patients and families regarding disease processes, treatments, and preventive care
- Ensure precise, timely documentation and compliance with regulatory standards
- Participate in case conferences, staff meetings, and continuing education
