An opportunity is available for an experienced and dedicated Registered Nurse to deliver skilled home health visits to patients needing compassionate, patient-centered care. This contract role is ideal for nurses who excel in managing clinical responsibilities independently and enjoy the flexibility of field-based work.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Field-based with daily travel within your assigned territory
• Caseload: 2532 points per week (typically 35 patient visits each day)

Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, including medication administration, wound care, IV therapy, and ongoing chronic disease management.
• Monitor patient progress, respond to changes in condition, and communicate promptly with the healthcare team and physicians.
• Provide comprehensive education to patients and their families regarding disease, treatments, medications, and preventive care.
• Ensure accurate, timely documentation using Home Care Home Base.
• Collaborate with therapists, medical social workers, and aides to provide coordinated care.
• Participate in meetings, case conferences, and required training or continuing education.
• Adhere to regulatory, agency, and compliance standards.

Qualifications:

• Current RN license (active and in good standing)
• Minimum of 1 year of recent home health experience (field-based strongly preferred)
• Proficiency with Home Care Home Base and OASIS documentation
• Strong clinical judgment, independence, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• BSN and prior experience managing a similar caseload preferred
• Familiarity with Medicare guidelines and compliance a plus

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
