A dedicated and compassionate Registered Nurse with home health experience is needed to deliver skilled, patient-focused care in a field-based role. This is a full-time, 13-week contract position. As a home health RN, youll conduct visits to patients residences, coordinate care, and play a critical role in supporting patient wellness and independence in the community.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments; develop individualized care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor clinical status, identify patient changes, and promptly communicate with physicians and the interdisciplinary care team.
• Provide thorough patient and family education on disease management, medications, and preventive care.
• Maintain detailed, timely electronic documentation in Home Care Home Base (HCHB).
• Collaborate seamlessly with PTs, OTs, medical social workers, and home health aides to ensure high-quality, coordinated care.
• Attend case conferences and staff meetings as needed; participate in ongoing education.
• Adhere to all regulatory and agency standards for documentation and patient care.

Qualifications:

• Active, unencumbered RN license.
• Minimum of one year of recent home health (field) experience.
• Proficiency in Home Care Home Base (HCHB) and completion of OASIS assessments.
• Exceptional clinical assessment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and up-to-date auto insurance.
• Strong time management and organizational abilities.
• BSN preferred.
• Previous experience managing 2532 points per week and knowledge of Medicare/home health compliance is ideal.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
