Deliver exceptional patient-centered care in a rewarding field-based environment as a Home Health RN. Youll visit patients in their homes, conduct skilled nursing assessments, and implement personalized care plans that make a meaningful impact on patient well-being.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Manage a caseload of 2532 points per week (35 patient visits per day)
• Daily travel throughout your assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Provide skilled home-based nursing care, including wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patient progress, identify changes in condition, and communicate promptly with the healthcare team and physicians.
• Educate patients and families on managing conditions, medications, treatment plans, and preventive care.
• Accurately document visits and care using Home Care Home Base (HCHB).
• Collaborate closely with therapists, social workers, and home health aides to ensure seamless coordination.
• Participate in case conferences, meetings, and ongoing professional development.
• Adhere to agency and regulatory standards for care delivery and documentation.

Qualifications:

• Active RN license in state of practice (in good standing)
• Minimum 1 year of recent field-based Home Health RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Strong clinical assessment, critical thinking, and documentation skills
• Reliable transportation, drivers license, and auto insurance
• Ability to work independently, prioritize tasks, and organize effectively
• Strong communication and interpersonal skills

Preferred:

• BSN degree
• Experience managing 2532 points per week
• Familiarity with Medicare and home health compliance
