A rewarding opportunity is available for a skilled and compassionate Registered Nurse with home health expertise. This contract position calls for a dedicated clinician to deliver high-quality, patient-centered nursing care in a field-based setting. You will ensure patients receive individualized care to foster comfort, healing, and positive outcomes at home.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Complete detailed OASIS assessments and craft tailored care plans for each patient
- Deliver skilled nursing interventionswound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress; identify, document, and report changes in condition promptly
- Educate patients and families about treatments, disease processes, medications, and preventive care
- Accurately chart all activities in Home Care Home Base (HCHB)
- Coordinate with physical/occupational therapists, social workers, and aides for a comprehensive care approach
- Attend case conferences, staff meetings, and pursue ongoing education as necessary
- Meet regulatory, quality, and documentation standards
Requirements:
- Active, unencumbered RN license in the state of practice
- Minimum of 1 year of current Home Health nursing experience
- Home Care Home Base (HCHB) and OASIS proficiency
- Strong clinical assessment skills, organized and independent work ethic
- Reliable vehicle, active drivers license, and car insurance
- Effective communication and time management
Preferred:
- BSN
- Experience managing a 2532-point caseload per week
- Familiarity with Medicare and compliance standards
