An opportunity is available for an experienced and compassionate Registered Nurse with a background in home health care. This position focuses on delivering high-quality skilled nursing services to patients in the comfort of their own homes. The role is perfect for nurses who thrive in independent, field-based environments and excel in patient-centered approaches.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• 13-week full-time contract

Key Responsibilities:

• Conduct OASIS assessments and develop individualized, goal-oriented care plans.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and quickly communicate any changes to the healthcare team and physicians.
• Educate patients and families on care plans, medications, and preventive health strategies.
• Document care and assessments accurately using Home Care Home Base (HCHB).
• Work in coordination with physical therapists, occupational therapists, medical social workers, and home health aides.
• Attend case conferences, participate in ongoing professional training, and uphold all agency and regulatory standards.

Required Qualifications:

• Active RN license (in good standing)
• Minimum 1 year of recent home health experience, with field-based background strongly preferred
• Proficiency in Home Care Home Base (HCHB) and experience with OASIS
• Strong assessment, critical thinking, and organizational abilities
• Reliable transportation, valid driver's license, and current auto insurance
• Comfortable working independently and managing a dynamic schedule

Preferred Qualifications:

• BSN degree
• Experience with Medicare standards and managing a caseload of 2532 points per week

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
