Job Description
Delivering high-quality, patient-focused care in a home health setting brings unique rewards and challenges. As a Home Health Registered Nurse, you will provide skilled nursing services to patients in their own homes, ensuring continuity, safety, and comfort while helping clients achieve their health goals. This 13-week contract offers the chance to showcase your clinical excellence, organizational talents, and compassionate approach as you serve the community.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based: travel within an assigned territory, with mileage reimbursement
- Caseload: 2532 points per week (typically 35 patient visits daily)
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Deliver a wide range of skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
- Evaluate patient status, monitor changes in condition, and communicate promptly with physicians and the care team.
- Educate patients and families on clinical procedures, treatment adherence, medications, and preventive care.
- Accurately document all care activities and patient interactions in Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and home health aides to coordinate all aspects of patient care.
- Attend case conferences and staff meetings, and participate in continuing education opportunities.
- Maintain compliance with regulatory requirements and agency standards.
Qualifications:
- Active state RN license (in good standing)
- Minimum 1 year of recent field-based home health RN experience
- Proficient in Home Care Home Base (HCHB) and OASIS documentation
- Strong assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage daily field responsibilities
- BSN and prior experience with Medicare compliance preferred
