Delivering high-quality, patient-focused care in a home health setting brings unique rewards and challenges. As a Home Health Registered Nurse, you will provide skilled nursing services to patients in their own homes, ensuring continuity, safety, and comfort while helping clients achieve their health goals. This 13-week contract offers the chance to showcase your clinical excellence, organizational talents, and compassionate approach as you serve the community.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: travel within an assigned territory, with mileage reimbursement
• Caseload: 2532 points per week (typically 35 patient visits daily)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver a wide range of skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient status, monitor changes in condition, and communicate promptly with physicians and the care team.
• Educate patients and families on clinical procedures, treatment adherence, medications, and preventive care.
• Accurately document all care activities and patient interactions in Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to coordinate all aspects of patient care.
• Attend case conferences and staff meetings, and participate in continuing education opportunities.
• Maintain compliance with regulatory requirements and agency standards.

Qualifications:

• Active state RN license (in good standing)
• Minimum 1 year of recent field-based home health RN experience
• Proficient in Home Care Home Base (HCHB) and OASIS documentation
• Strong assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage daily field responsibilities
• BSN and prior experience with Medicare compliance preferred
