An exciting opportunity is available for an experienced and compassionate Registered Nurse skilled in Home Health to provide high-quality patient-centered care in a field-based setting. This position focuses on skilled nursing visits to patient homes, ensuring continuity of care and positive clinical outcomes.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Daily travel required within the assigned territory (mileage reimbursement provided)
Qualifications:
- Active Registered Nurse license in good standing
- At least 1 year of recent field-based Home Health experience
- Proficient with Home Care Home Base (HCHB) electronic documentation system
- Proven experience conducting OASIS assessments
- Excellent clinical assessment, time management, and organizational abilities
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently and maintain effective communication with the care team
- BSN preferred
- Familiarity with managing 2532 weekly points and Medicare/home health compliance is a plus
Key Responsibilities:
- Conduct comprehensive OASIS assessments, develop, and implement individualized care plans
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress and communicate changes to physicians and interdisciplinary team members
- Educate patients and families on treatment plans, disease management, and medications
- Document care promptly and accurately within the HCHB system
- Collaborate with therapists, social workers, and aides to coordinate services
- Participate in staff meetings, case conferences, and ongoing education
- Adhere to organizational and regulatory standards for documentation and quality of care
