Experienced and compassionate RNs are sought to deliver high-quality patient care in a home health setting through a 13-week full-time contract. This role centers on promoting health, independence, and positive outcomes for patients while enabling you to showcase critical thinking and independence as a clinician within the community.

Schedule Details:

• Monday Friday, 8:00 AM 5:00 PM
• Expectation of 2532 points per week, typically involving 35 patient visits per day
• Field-based, requiring daily local travel (mileage reimbursement provided)

Core Responsibilities:

• Complete comprehensive OASIS assessments and craft individualized, evidence-based care plans
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
• Track and evaluate patient progress, quickly identify clinical changes, and collaborate with physicians and interdisciplinary team members
• Educate patients and families about diagnoses, medications, and preventive strategies
• Maintain detailed and prompt documentation using Home Care Home Base (HCHB)
• Participate in case conferences and continuing education opportunities
• Ensure compliance with regulatory and agency documentation and care delivery standards

Qualifications:

• Active, unencumbered RN license for the state of practice
• At least one year of recent home health field experience
• Proficiency utilizing Home Care Home Base (HCHB) and OASIS
• Strong clinical assessment, communication, and organizational skills
• Ability to work independently and adapt to a dynamic field-based schedule
• Reliable personal transportation, valid drivers license, and current auto insurance
• BSN and experience with Medicare compliance or a caseload of 2532 points weekly are preferred

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
