An exciting opportunity is available for an experienced Home Health Registered Nurse (RN) to deliver high-quality patient care in the home setting. This is a full-time, 13-week contract, ideal for motivated clinicians who excel at working independently and thrive in a dynamic field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans for home health patients.
• Perform skilled nursing tasks, including wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient progress, recognize and respond to changes in condition, and communicate promptly with the multidisciplinary team and physicians.
• Educate patients and families on disease management, treatment protocols, and prevention strategies.
• Utilize Home Care Home Base (HCHB) for timely and accurate documentation.
• Collaborate with physical/occupational therapists, social workers, and home health aides to ensure continuity and quality of care.
• Participate in case conferences, team meetings, and ongoing professional development.
• Adhere to all regulatory and compliance standards in home health services.
• Complete daily field visits, with a caseload of 2532 points per week (typically 35 patients per day).

Qualifications:

• Active RN license (in good standing)
• Minimum one year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments (required)
• Strong clinical assessment, organizational, and communication skills
• Reliable transportation, current drivers license, and auto insurance
• BSN preferred
• Experience with Medicare guidelines and managing higher point caseloads is highly valued

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
