This contract position offers you the chance to provide skilled, patient-centered care directly in patients homes. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Highlights:
- 13-week full-time contract
- Caseload: 2532 points per week (typically 35 visits per day)
- Field-based: Daily travel to patients homes within your assigned territory
Key Responsibilities:
- Complete comprehensive OASIS assessments and design individualized care plans
- Deliver skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management
- Assess patient progress, monitor for changes, and collaborate promptly with the interdisciplinary care team
- Educate patients and families on disease processes, medication management, and health prevention strategies
- Accurately document all care and updates in Home Care Home Base (HCHB)
- Coordinate with physical and occupational therapists, medical social workers, and home health aides
- Attend case conferences and participate in ongoing staff education to ensure excellent patient outcomes
Qualifications:
- Active RN license in good standing in the state of practice
- At least 1 year of recent experience in Home Health (field experience strongly preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS essential
- Strong clinical assessment, time-management, and communication skills
- Reliable transportation, valid drivers license, and auto insurance required
- BSN preferred and previous caseload management of 2532 points per week is a plus
- Familiarity with Medicare and home health compliance a bonus
