Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse, supporting individuals in their homes and helping them achieve optimal wellness. This field-based position is suited for compassionate nurses who thrive in autonomous settings and demonstrate a commitment to clinical excellence.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM to 5:00 PM
- Caseload: 2532 points per week (approximately 35 patient visits daily)
- Daily travel within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct thorough OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, promptly report changes in condition, and coordinate with physicians and the interdisciplinary care team.
- Educate patients and their families on diagnoses, medications, care plans, and preventive strategies.
- Document clinical notes accurately and efficiently using Home Care Home Base (HCHB).
- Collaborate with therapists, medical social workers, and home health aides to ensure comprehensive care.
- Participate in case conferences, ongoing staff meetings, and continuing education.
- Stay in compliance with clinical, documentation, and regulatory standards.
Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent home health RN experience (field experience preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS assessment tools
- Demonstrated clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and appropriate auto insurance
- Independent, well-organized, and adaptable in a field-based schedule
- Excellent communication, time management, and organizational abilities
- BSN and prior experience managing 2532 points per week preferred
- Familiarity with Medicare and home health compliance standards preferred
