Job Description
A skilled and compassionate Registered Nurse with recent home health experience will excel in this field-based contract opportunity. Responsible for delivering patient-centered care in the home setting, youll play a vital role in ensuring quality outcomes, providing a personal touch, and supporting patient independence.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based care with daily travel (mileage reimbursement provided)
- Caseload expectation: 2532 points per week (roughly 35 patient visits per day)
Key Responsibilities:
- Perform comprehensive OASIS assessments and continuously update care plans
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, document changes, and communicate promptly with the interdisciplinary team and physicians
- Educate patients and their families about disease processes, treatment protocols, medications, and preventative care
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Coordinate care with therapists, medical social workers, and home health aides
- Participate in case conferences, staff meetings, and ongoing professional development
- Uphold regulatory and agency standards for safe, effective home health care
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health field experience
- Proficient in Home Care Home Base (HCHB) and OASIS
- Strong clinical assessment, organizational, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Independent, self-motivated, and attentive to detail
Preferred:
- BSN degree
- Prior management of a 2532 point caseload
- Familiarity with Medicare compliance in home health settings
