An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes. This role focuses on ensuring quality, continuity, and patient-centered care while working independently within a structured support system.

Schedule and Contract Type:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate promptly with the care team and physicians.
- Educate patients and families on treatment plans, medications, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary teams including physical therapists, occupational therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
- Adhere to regulatory and agency standards for documentation and care delivery.
Qualifications & Desired Experience:
- Active RN license in good standing within the state of practice.
- Minimum of 1 year recent Home Health experience, preferably field-based.
- Proficiency in Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment and critical thinking capabilities.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, organizational, and time management skills.
- BSN preferred.
- Experience managing 2532 points per week is a plus.
- Knowledge of Medicare and home health compliance standards advantageous.
