Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is sought to provide skilled nursing care to patients in their homes, delivering quality, continuity, and patient-centered care. This position requires a self-motivated and organized professional able to work independently while upholding high clinical standards and effective patient communication.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate promptly with care teams and physicians.
  • Educate patients and families on disease processes, treatments, medications, and preventive care.
  • Maintain precise and timely documentation using Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with all regulatory and agency standards for documentation and patient care.

Qualifications & Desired Experience:

  • Active RN license in good standing for the state of practice.
  • Minimum one year of recent Home Health experience, preferably field-based.
  • Proficient in Home Care Home Base (HCHB) and OASIS assessment tools.
  • Strong clinical assessment and critical thinking skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently within a flexible, field-based schedule.
  • Excellent communication, time management, and organizational abilities. Preferred:
  • Bachelor of Science in Nursing (BSN).
  • Experience managing 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.

Schedule & Location:

  • Full-time, 13-week contract.
  • MondayFriday, 8:00 AM to 5:00 PM.
  • Caseload typically involves 35 patient visits per day (2532 points weekly).
  • Daily travel within assigned territory with mileage reimbursement.
  • Field-based care in patient homes.
