Deliver skilled, compassionate nursing care directly in patients homes as a Home Health Registered Nurse (RN) within a flexible, supportive contract role. This position requires strong clinical expertise and independence to provide high-quality patient-centered care while managing an efficient caseload.

Role Highlights:
- Full-time, 13-week contract position
- Schedule: MondayFriday, 8:00 AM to 5:00 PM
- Caseload: 2532 points per week (approximately 35 patient visits per day)
- Field-based: travel daily to patient homes within assigned territory with mileage reimbursement
- Utilize Home Care Home Base (HCHB) electronic documentation and OASIS assessment tools
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify changes, and communicate promptly with the interdisciplinary team and physicians
- Educate patients and families on treatment, medications, and preventive care
- Collaborate closely with therapists, social workers, and aides to ensure coordinated, seamless care
- Maintain accurate and timely documentation adhering to regulatory standards
Qualifications and Experience:
- Active RN license in good standing within the state of practice
- Minimum of 1 year recent home health nursing experience (field experience preferred)
- Proficient with Home Care Home Base (HCHB) and knowledgeable in OASIS assessments
- Reliable transportation, valid drivers license, and insurance
- Strong assessment, critical thinking, communication, and organizational skills
- BSN preferred; knowledge of Medicare and home health compliance is a plus
