An opportunity for a skilled Home Health Registered Nurse (RN) is available for a full-time 13-week contract, providing patient-centered care in a field-based setting. This role requires delivering comprehensive nursing services in patient homes, ensuring the highest standards of clinical practice, patient education, and coordinated care.

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans tailored to patient needs.
• Administer skilled nursing care, including wound care, medication management, IV therapy, and chronic disease oversight.
• Monitor patient progress, recognize changes in condition, and communicate promptly with care teams and physicians.
• Educate patients and families about disease management, medications, and preventive health.
• Maintain detailed and timely documentation using Home Care Home Base (HCHB) electronic records.
• Collaborate with interdisciplinary professionals such as therapists, social workers, and home health aides to ensure integrated care delivery.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Comply with all regulatory and agency standards concerning patient care and documentation.

Qualifications and Desired Experience:

• Active RN license in good standing within the state of practice.
• Minimum of one year recent home health nursing experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools is required.
• Strong clinical assessment, critical thinking, and communication skills.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work autonomously and manage a flexible, patient-focused schedule.
• Bachelor of Science in Nursing (BSN) preferred.
• Experience managing a caseload of 2532 points weekly.
• Familiarity with Medicare regulations and home health compliance standards is advantageous.

Schedule:

• MondayFriday, 8:00 AM to 5:00 PM
• Caseload of approximately 35 patients daily (2532 points per week)

This contract position requires daily travel to patient homes within the assigned territory, with mileage reimbursement provided.
