Healthcare professionals passionate about patient-centered care will thrive as a Home Health Registered Nurse delivering skilled nursing services to individuals in their homes. This 13-week contract position offers the opportunity to make a meaningful impact by supporting clients through every step of their health journey.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Administer skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, identify changes, and communicate updates to the healthcare team and physicians
- Educate patients and families about disease processes, medications, and preventive health
- Document care accurately and promptly using Home Care Home Base (HCHB)
- Coordinate care with physical therapists, occupational therapists, social workers, and home health aides
- Participate in case conferences, staff meetings, and ongoing education opportunities
- Adhere to all agency and regulatory standards in care delivery and documentation
Qualifications:
- Active RN license (in good standing) for the state of practice
- At least 1 year of recent field experience in home health nursing
- Proficiency in Home Care Home Base (HCHB) required
- OASIS assessment tool experience required
- Strong clinical assessment, communication, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a field-based schedule
- BSN preferred; knowledge of Medicare/home health standards highly valued
- Previous experience managing a caseload of 2532 points/week preferred
