Take your nursing expertise into the field and make a direct impact in patient lives as a Home Health RN. This full-time contract role focuses on delivering exceptional skilled nursing care to patients in the comfort of their homes, supporting quality outcomes while working closely with interdisciplinary healthcare teams. You'll manage a flexible caseload, shape individualized care plans, and educate familiesall while enjoying professional autonomy and a rewarding work environment.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Daily local travel to patient homes (mileage reimbursement provided)

Responsibilities:

• Complete comprehensive OASIS assessments and design tailored care strategies
• Deliver direct skilled nursing services: wound care, IV therapy, medication management, and chronic disease support
• Monitor patient progress and report any changes promptly to the healthcare team
• Educate patients and families on care plans, medications, and safe practices
• Accurately document all care using Home Care Home Base (HCHB)
• Collaborate with therapists, social workers, and aides for holistic patient care
• Participate in case conferences, staff meetings, and required trainings
• Ensure adherence to regulatory and agency standards

Required Qualifications:

• Active RN Licensure in good standing
• Minimum 1 year of recent Home Health experience (preferably field-based)
• Proficiency with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, current auto insurance
• Independent, organized, and strong communicator
• Excellent clinical assessment and critical thinking skills

Preferred:

• BSN degree
• Familiarity with managing 2532 points per week
• Knowledge of Medicare and home health compliance standards

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
