Deliver skilled, compassionate nursing care directly to patients in their homes with this Home Health RN opportunity. This contract role centers on providing comprehensive assessments, individualized care plans, and fostering outstanding patient outcomes in a field-based environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct thorough OASIS assessments and develop tailored care plans.
- Administer skilled nursing care, including medication administration, wound care, IV therapy, and chronic disease management.
- Track and document patient progress, identifying changes and reporting to physicians and interdisciplinary teams.
- Educate patients and caregivers on medications, treatment plans, health conditions, and preventive care strategies.
- Maintain up-to-date documentation within Home Care Home Base (HCHB).
- Collaborate with therapists, home health aides, and social workers for coordinated, patient-centered care.
- Attend case conferences, staff meetings, and required training or continuing education.
- Adhere to regulatory and best practice standards for home health care.
Required Skills and Experience:
- Active RN license in good standing
- Minimum of one year recent home health nursing experience (field experience strongly preferred)
- Proficiency with OASIS and Home Care Home Base (HCHB)
- Excellent clinical assessment and critical thinking abilities
- Strong communication, organization, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
Preferred Qualifications:
- BSN degree
- Previous experience managing a caseload of 2532 points per week
- Understanding of Medicare and home health compliance
