An exciting opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care directly to patients in the comfort of their homes. This contract position is ideal for a self-driven professional with strong clinical skills and a commitment to fostering positive patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and craft individualized care plans based on patient needs.
• Deliver a variety of skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, identify changes in condition, and communicate with the healthcare team and physicians.
• Provide patient and family education on disease management, medications, treatments, and preventive care.
• Ensure documentation is accurate and timely within the Home Care Home Base (HCHB) system.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to deliver coordinated care.
• Attend case conferences, participate in staff meetings, and engage in ongoing professional education.
• Comply with regulatory and agency standards for all documentation and patient care.

Qualifications:

• Active RN License in good standing.
• At least 1 year of recent Home Health nursing experience (field-based preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, critical thinking, and organizational skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently, maintain a flexible schedule, and manage a point-based caseload.
• BSN and experience managing 2532 points per week preferred.
• Knowledge of Medicare and home health compliance standards a plus.
