Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes throughout the Pullman, WA area. This field-based role offers the opportunity to deliver patient-centered, high-quality care while working independently within a flexible schedule.
Responsibilities:
- Conduct comprehensive OASIS assessments; develop and manage individualized care plans.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate promptly with the interdisciplinary care team and physicians.
- Educate patients and families on disease processes, treatment plans, medications, and preventive care.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic system.
- Collaborate with therapists, medical social workers, and home health aides to ensure coordinated care delivery.
- Participate in case conferences, staff meetings, and continuing education.
- Adhere strictly to regulatory standards and agency policies.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum 1 year of recent home health experience, preferably with field visits.
- Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments.
- Strong clinical assessment, critical thinking, organizational, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to effectively manage a flexible field-based schedule and work independently.
- BSN preferred.
- Experience managing caseloads of 2532 points per week and knowledge of Medicare/home health compliance is a plus.
Contract Details:
- Full-time, 13-week contract.
- Schedule: Monday Friday, 8:00 AM 5:00 PM.
- Caseload: 2532 points per week (approx. 35 patient visits daily).
- Mileage reimbursement included.
