Job Description
Deliver skilled, patient-focused nursing care in the comfort of patients homes as an experienced Home Health Registered Nurse. This dynamic role centers on providing individualized, high-quality care and supporting recovery and wellbeing in the home environment. The position is structured as a full-time, 13-week contract with opportunity for extension, designed for professionals who thrive in autonomous settings and value collaboration within a multidisciplinary team.
Schedule and Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based; daily travel to patient homes within assigned territory (mileage reimbursement provided)
- Expected caseload: 2532 points per week (typically 35 patient visits per day)
- Electronic documentation via Home Care Home Base (HCHB)
- OASIS assessments required
Responsibilities:
- Conduct comprehensive OASIS assessments and develop tailored care plans.
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient status, identify changes in condition, and communicate with healthcare team and physicians.
- Educate patients and caregivers on disease management, medications, and preventive strategies.
- Accurately document all care and assessments in HCHB system.
- Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in case conferences, staff meetings, and relevant training sessions.
- Comply with all regulatory, documentation, and patient care standards.
Qualifications:
- Active RN license, in good standing, in the state of practice.
- At least 1 year of recent home health experience (field experience preferred).
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Reliable transportation, valid drivers license, and auto insurance.
- Strong clinical skills, ability to work independently, and effective communication.
Preferred background includes a BSN, prior experience managing similar caseloads, and knowledge of Medicare/home health compliance standards.
