Key Responsibilities:

• Conduct comprehensive patient assessments in the home setting
• Develop, implement, and evaluate individualized care plans
• Administer medications, wound care, and treatments as prescribed
• Collaborate with multidisciplinary teams to ensure seamless patient care
• Educate patients and family members on illness management, medications, and overall health
• Maintain accurate and timely clinical documentation in compliance with regulatory requirements
• Monitor patient progress and adjust nursing interventions as needed
• Communicate effectively with physicians, therapists, and other care professionals

Desired Qualifications:

• Current RN license in good standing
• Previous experience in home health nursing strongly preferred
• Solid clinical assessment and critical thinking abilities
• Proficiency with electronic medical records and documentation
• Excellent communication, organizational, and time management skills
• Ability to work autonomously and adapt to varying home environments
• Reliable transportation and valid drivers license

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
