Job Description
Deliver exceptional care as a Home Health Registered Nurse, working one-on-one with patients in the comfort of their homes. This contract offers a dynamic Monday through Friday schedule, 8:00 AM 5:00 PM, serving a caseload of 2532 points per weekwhich typically equates to about 35 patient visits each day. You will travel within your assigned territory, with mileage reimbursement provided, using the Home Care Home Base (HCHB) electronic documentation system and conducting OASIS assessments.
Key Qualifications:
- Active RN license in good standing in Pennsylvania
- At least 1 year of recent home health nursing experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB)
- OASIS assessment experience required
- Strong clinical assessment, communication, and documentation skills
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivation and the ability to work independently
Preferred Experience:
- Bachelor of Science in Nursing (BSN)
- Previous management of a 2532 point per week caseload
- Familiarity with Medicare and home health compliance standards
Position Responsibilities:
- Perform comprehensive OASIS assessments and implement individualized care plans
- Deliver skilled nursing care: wound care, IV therapy, medication administration, chronic disease management
- Communicate changes in patient condition to interdisciplinary teams and physicians
- Educate patients and families regarding care plans, medications, and disease management
- Maintain comprehensive and timely documentation in HCHB
- Collaborate with therapists and allied health professionals to ensure coordinated patient care
- Participate in staff meetings, case conferences, and ongoing training
