Deliver patient-centered care and make a meaningful impact as a Home Health Registered Nurse providing skilled nursing support to individuals in their homes. This full-time, 13-week contract position focuses on high-quality, compassionate care, continuity, and clinical excellence for a diverse patient population within a supportive, collaborative environment.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Position Highlights:

• Manage a caseload of 2532 points per week (typically 35 patient visits per day)
• Field-based setting (daily travel required, mileage reimbursement provided)
• Utilize Home Care Home Base (HCHB) for documentation
• Perform OASIS assessments as part of patient care

Key Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop individualized care plans
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, promptly identify changes, and communicate with physicians and care teams
• Educate patients and families regarding diseases, treatments, medications, and prevention strategies
• Ensure timely and accurate documentation in HCHB
• Collaborate with interdisciplinary teams, including therapists and home health aides, to coordinate care
• Attend case conferences, staff meetings, and participate in ongoing education
• Adhere to regulatory and agency standards for home health care

Qualifications:

• Active RN License in good standing
• At least 1 year of recent home health experience (field experience preferred)
• Proficiency in Home Care Home Base (HCHB)
• Strong clinical assessment, critical thinking, and communication skills
• OASIS experience required
• Valid drivers license, reliable transportation, and auto insurance
• Ability to work independently and manage a flexible schedule
• BSN and Medicare/home health compliance knowledge preferred
