Deliver high-quality, patient-centered care as a Home Health Registered Nurse, supporting individuals in their own homes. This field-based position calls for an RN with a passion for autonomy, strong organizational skills, and a commitment to maintaining clinical excellence across every visit. Youll play an essential part in improving patient outcomes through comprehensive assessment, direct skilled nursing care, and collaboration with a dedicated interdisciplinary health team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and create individualized care plans.
• Deliver expert skilled nursing, including wound care, medication administration, IV therapy, and management of chronic conditions.
• Monitor patient status, identify changes, and communicate promptly with physicians and care teams.
• Educate patients and families on diagnoses, treatments, medications, and preventive care.
• Maintain precise and timely electronic documentation in Home Care Home Base (HCHB).
• Coordinate closely with therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and professional development activities.
• Ensure compliance with all regulatory and agency standards.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent field-based Home Health RN experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Excellent clinical assessment and critical thinking capabilities.
• Well-developed communication, time management, and organizational skills.
• Reliable transportation, a valid drivers license, and auto insurance.
• BSN preferred.
• Prior experience managing a caseload of 2532 points per week is a plus.
• Knowledge of Medicare and home health compliance standards is beneficial.
