Job Description

An experienced Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes. This role involves comprehensive patient assessments, individualized care planning, coordination with interdisciplinary teams, and patient/family education to ensure quality, patient-centered care. The ideal candidate operates independently with strong clinical skills, exceptional communication, and reliable time management.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify condition changes, and communicate effectively with care teams and physicians.
  • Educate patients and families about disease processes, treatments, medications, and preventive care.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and aides for coordinated care.
  • Participate in case conferences, staff meetings, and continuing education.
  • Adhere strictly to regulatory and agency standards for patient care and documentation.

Qualifications and Desired Experience:

  • Active RN license in good standing within the state.
  • Minimum of 1 year recent experience in home health nursing (field experience preferred).
  • Proficiency in Home Care Home Base (HCHB) and OASIS assessments required.
  • Strong clinical assessment, critical thinking, and organizational skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to manage a flexible field-based schedule independently.
  • BSN preferred.
  • Experience managing 2532 weekly points and knowledge of Medicare/home health compliance standards beneficial.

Job Details:

  • Contract Type: Full-time, 13-week contract.
  • Schedule: Monday through Friday, 8:00 AM 5:00 PM.
  • Caseload: 2532 points weekly (typically 35 patient visits daily).
  • Travel: Daily travel to patient homes within assigned territory; mileage reimbursement provided.
  • Documentation System: Home Care Home Base (HCHB).
  • Assessment Tool: OASIS.
