Job Description
A skilled and compassionate Home Health Registered Nurse (RN) is needed to provide exceptional in-home care for patients. This 13-week contract position is ideal for an adaptable nurse who thrives in an autonomous, patient-centered environment, blending clinical expertise with outstanding communication and organizational skills.
Schedule & Structure:
- Full-time, Monday Friday, 8:00 AM 5:00 PM
- 13-week contract duration
- Caseload: 2532 points per week (35 patient visits daily)
- Daily travel to patient homes within the assigned territory (mileage reimbursement provided)
- Field-based setting
Responsibilities:
- Complete comprehensive OASIS assessments and craft individualized care plans
- Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, recognize condition changes, and coordinate promptly with physicians and interdisciplinary teams
- Educate patients and families on care plans, medications, and preventive health
- Maintain precise and timely electronic documentation in Home Care Home Base (HCHB)
- Participate in care conferences, staff meetings, and required ongoing education
- Ensure compliance with regulatory and agency patient care standards
Qualifications:
- Active RN license in good standing (state of practice)
- At least 1 year of recent home health (field) experience
- Familiarity with Home Care Home Base (HCHB) and OASIS assessment tool required
- Reliable transportation, drivers license, and auto insurance
- Strong assessment, time management, and communication abilities
- Ability to work independently and adapt to a flexible schedule
Preferred:
- BSN
- Proven experience with 2532 point caseloads and knowledge of Medicare/home health regulations
