A rewarding opportunity awaits an experienced and compassionate Registered Nurse with a strong background in home health. You will provide high-quality nursing care to patients in their own homes, ensuring individualized care and promoting optimal health outcomes. This full-time, 13-week contract position is ideal for self-motivated professionals who thrive working independently while maintaining a patient-centered approach.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop tailored care plans
- Deliver skilled nursing care: wound management, medication administration, IV therapy, and chronic disease oversight
- Monitor and document patient progress, identifying changes and communicating with the healthcare team and physicians as needed
- Educate patients and families regarding diagnoses, treatments, medications, and preventive strategies
- Maintain accurate documentation in the Home Care Home Base (HCHB) system
- Collaborate with the interdisciplinary team (PT, OT, MSW, home health aides) to ensure coordinated, high-quality care
- Attend case conferences and staff meetings, and participate in ongoing professional development
- Adhere to all agency and regulatory standards for patient care and documentation
Qualifications:
- Active RN license (in good standing) for the state of practice
- Minimum 1 year of recent home health experience (field-based roles strongly preferred)
- Expertise in Home Care Home Base (HCHB) and OASIS assessment tools
- Excellent clinical assessment, critical thinking, and communication abilities
- Reliable transportation, current drivers license, and insurance
- BSN preferred; experience managing a 2532 point caseload and Medicare/home health compliance knowledge is a plus
