Key Qualifications:

• Current, unrestricted RN license
• Previous home health nursing experience required
• Strong assessment and critical thinking abilities
• Proficient in developing and updating patient care plans
• Excellent communication and documentation skills
• Ability to work independently, managing a flexible caseload
• Current BLS certification

Responsibilities:

• Conduct comprehensive in-home patient assessments to determine healthcare needs
• Collaborate with healthcare teams to establish and adjust individualized care plans
• Administer prescribed medications, monitor vital signs, and document changes in condition
• Educate patients and their families on ongoing treatments and self-care techniques
• Coordinate with physicians, therapists, and other professionals to ensure a cohesive care approach
• Maintain accurate clinical records in compliance with all regulatory requirements
• Adhere to infection control policies and patient safety protocols during all interactions

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
