An opportunity is available for a skilled and compassionate Registered Nurse with home health experience to support patients in their homes. This role requires autonomy, strong clinical expertise, and a commitment to patient-centered care. You'll play a vital part in the healthcare team by ensuring high-quality outcomes for a diverse patient population.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans tailored to each patients needs.
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient conditions, evaluating progress and promptly updating interdisciplinary teams and physicians.
• Educate patients and families on diagnoses, treatment protocols, medication administration, and preventive health strategies.
• Accurately document all care and updates in Home Care Home Base (HCHB).
• Collaborate closely with therapists, social workers, and home health aides for coordinated, effective care.
• Participate in case conferences and staff meetings, engaging in ongoing professional development.
• Ensure compliance with regulatory standards and all reporting requirements.
• Manage a caseload of 2532 points per week (typically 35 patient visits per day), providing vital care throughout the assigned territory.

Qualifications:

• Active RN license (state-specific, in good standing).
• Minimum one year of recent field-based home health RN experience.
• Proficient with Home Care Home Base (HCHB) electronic documentation.
• Solid experience using OASIS for assessments and care planning.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Excellent assessment and critical thinking skills with strong time management abilities.
• BSN preferred.
• Prior experience with similar caseloads and understanding of Medicare/home health compliance is a plus.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
