Job Description
Bring your clinical expertise as a Home Health Registered Nurse (RN) providing high-quality care and support to patients in their homes. Youll manage a flexible field-based schedule in a contract position focused on delivering patient-centered, skilled nursing services, contributing to ongoing patient recovery and wellbeing.
Key Details:
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Contract Length: 13 weeks, full-time
- Caseload: 2532 points per week (generally 35 patient visits daily)
- Setting: Patient homes within an assigned territory
- Daily travel required (mileage reimbursement provided)
- Electronic documentation: Home Care Home Base (HCHB)
- Assessment tool: OASIS
Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor changes in patient conditions and coordinate necessary interventions with the healthcare team
- Educate patients and families on diagnoses, treatments, and preventive strategies
- Ensure accurate, timely documentation in HCHB
- Collaborate closely with physical therapists, occupational therapists, social workers, and aides
- Participate in case conferences, staff meetings, and ongoing professional development
- Uphold regulatory and agency policies for care and documentation
Qualifications:
- Active RN license (in good standing)
- At least one year of recent field-based home health RN experience
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Solid clinical assessment and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage routine travel
- BSN and experience handling 2532 points weekly preferred
- Familiarity with Medicare and home health compliance standards a plus
