Seeking an experienced and compassionate Registered Nurse to deliver skilled nursing care to patients in their homes. This 13-week contract position offers the opportunity to make a direct impact on patient well-being by supporting individuals and families through home health services. The role emphasizes autonomy, strong clinical judgment, and effective communication to ensure quality patient-centered outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Field-based setting, with daily travel to patient homes

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Administer skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patient progress, reporting any changes to the care team and coordinating follow-ups.
• Educate patients and families on disease management, treatment plans, and health maintenance.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Ensure compliance with Medicare standards and agency protocols.

Qualifications:

• Active RN license (in good standing)
• Minimum of one year of recent home health experience (field-based preferred)
• Proficiency with HCHB and experience with OASIS assessments
• Reliable vehicle, valid drivers license, and current auto insurance
• Strong assessment, communication, and time management skills
• BSN preferred; experience managing 2532 points per week is a plus
• Familiarity with Medicare/home health compliance preferred
