An opportunity is available for an experienced and compassionate Registered Nurse dedicated to home health care. This role is ideal for an RN who excels at providing skilled nursing services in a patients home while maintaining clinical excellence, strong communication, and meticulous documentation.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Duration:

• Full-time, 13-week contract

Caseload:

• 2532 points per week (approximately 35 patient visits per day)
• Daily travel with mileage reimbursement provided

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient progress, promptly communicate changes in condition, and coordinate care with interdisciplinary teams
• Educate patients and family members on diagnoses, treatments, medications, and preventive care
• Accurately document all patient interactions in Home Care Home Base (HCHB)
• Collaborate with therapy staff, social workers, and home health aides to ensure well-coordinated care delivery
• Attend case conferences and staff meetings and participate in ongoing education

Qualifications:

• Active RN license, in good standing
• At least 1 year of recent home health field experience
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Excellent assessment, communication, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to manage a flexible, independent schedule
• BSN preferred; experience managing high-point caseload and understanding Medicare/home health compliance are pluses
