Job Description

Deliver quality, compassionate care as a skilled Home Health Registered Nurse, providing vital services to patients within the comfort of their own homes. This role calls for an experienced RN who thrives in a field-based, autonomous environment, collaborating closely with interdisciplinary teams to ensure continuity and excellence in patient-centered care.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM

Responsibilities include:

  • Conducting comprehensive OASIS assessments and developing individualized plans of care.
  • Delivering skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitoring patient conditions, communicating timely updates to care teams and physicians.
  • Educating patients and families regarding disease management, medications, and preventive strategies.
  • Accurately documenting visits and interventions using Home Care Home Base (HCHB).
  • Coordinating with therapists, social workers, and home health aides for well-rounded care.
  • Participating in case conferences, staff meetings, and ongoing education.
  • Ensuring compliance with regulatory and agency care standards.

Required qualifications:

  • Active, unencumbered RN license in the applicable state.
  • At least one year of recent Home Health nursing experience, with field experience highly valued.
  • Demonstrated proficiency in HCHB and OASIS.
  • Strong clinical assessment, independent decision-making, and organizational abilities.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Effective communication and interpersonal skills.

Preferred:

  • BSN degree.
  • Experience in managing a caseload of 2532 points weekly.
  • Knowledge of Medicare and home health compliance standards.
