Join a dedicated healthcare team as a Home Health Registered Nurse (RN) providing essential in-home care to patients in Plano, TX. This full-time, 13-week contract position offers the chance to deliver personalized, skilled nursing care that supports patient independence and promotes positive health outcomes in a community setting.
In this dynamic role, you will conduct comprehensive assessments using OASIS tools, develop tailored care plans, and deliver a range of nursing services including wound care, medication administration, IV therapy, and chronic disease management. You will closely monitor patient progress, communicate changes to interdisciplinary teams, and provide patient and family education to empower health management. Documentation is completed via Home Care Home Base (HCHB), ensuring compliance with agency and regulatory standards.
Key Responsibilities:
- Perform thorough OASIS assessments and create individualized care plans
- Deliver skilled nursing services in patients homes, ensuring high-quality care
- Monitor patient conditions and coordinate with physicians and care team members
- Educate patients and families on treatment, disease management, and preventive care
- Accurately document all nursing activities in HCHB
- Collaborate with therapists, social workers, and aides for seamless care coordination
- Participate in case conferences, staff meetings, and continuing education opportunities
Qualifications:
- Active RN license in good standing for Texas
- Minimum 1 year recent home health nursing experience with fieldwork preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments required
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation with valid drivers license and auto insurance
- Ability to work independently with excellent time management and organizational skills
Preferred:
- BSN degree
- Experience managing caseloads of 2532 points weekly
- Familiarity with Medicare and home health compliance requirements
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- Typically 35 patient visits daily with daily travel within assigned territory (mileage reimbursed)
