An experienced and compassionate Registered Nurse is needed for a dynamic home health contract assignment. In this field-based role, you'll deliver skilled nursing care directly to patients in the comfort of their own homes, ensuring continuity, excellence, and a patient-focused approach. The position best suits self-motivated RNs who thrive independently, possess strong organizational abilities, and are dedicated to maintaining clear communication with patients and healthcare teams.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: 2532 points per week (roughly 35 patient visits daily)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop patient-centered care plans.
• Deliver skilled interventions: wound care, IV therapy, chronic disease management, and medication administration.
• Evaluate patient status, promptly communicate changes to physicians and team members.
• Educate patients and families on diagnoses, medication, and preventive health practices.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members to ensure coordinated, quality care.
• Participate in meetings, case conferences, and ongoing training as necessary.
• Consistently adhere to regulations and agency standards.

Qualifications:

• Active RN License (in good standing).
• At least 1 year of recent field-based home health RN experience.
• Proficiency with OASIS and Home Care Home Base (HCHB) required.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Strong clinical, communication, and time-management skills.
• Ability to manage a flexible, field-based workload independently.
• BSN and Medicare compliance knowledge preferred.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
