Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes. This role emphasizes clinical excellence and patient-centered care delivered independently within a field-based setting. The successful candidate will conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to support positive patient outcomes.
Key Responsibilities:
- Conduct thorough OASIS assessments and create tailored care plans.
- Deliver nursing care including wound management, medication administration, IV therapy, and chronic disease monitoring.
- Monitor patient progress, recognize condition changes, and communicate promptly with physicians and care teams.
- Educate patients and families about disease management, medications, and preventive measures.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB) electronic records.
- Coordinate with therapists, social workers, and aides to ensure comprehensive care delivery.
- Participate in case conferences, clinical meetings, and ongoing education.
- Comply with regulatory standards and agency policies.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum of 1 year recent home health experience, preferably field-based.
- Proficient in Home Care Home Base (HCHB) and OASIS assessment tool.
- Strong clinical judgment and critical thinking skills.
- Reliable transportation, valid driver's license, and auto insurance.
- Ability to work independently and efficiently manage a flexible schedule.
- Excellent communication, organization, and time management skills.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing 2532 points weekly caseload.
- Familiarity with Medicare and home health compliance standards.
Schedule & Contract Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM to 5:00 PM.
- Typically 35 patient visits per day, with 2532 points per week.
- Mileage reimbursement provided.
