An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes. This role emphasizes clinical excellence and patient-centered care delivered independently within a field-based setting. The successful candidate will conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to support positive patient outcomes.

Key Responsibilities:

• Conduct thorough OASIS assessments and create tailored care plans.
• Deliver nursing care including wound management, medication administration, IV therapy, and chronic disease monitoring.
• Monitor patient progress, recognize condition changes, and communicate promptly with physicians and care teams.
• Educate patients and families about disease management, medications, and preventive measures.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB) electronic records.
• Coordinate with therapists, social workers, and aides to ensure comprehensive care delivery.
• Participate in case conferences, clinical meetings, and ongoing education.
• Comply with regulatory standards and agency policies.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent home health experience, preferably field-based.
• Proficient in Home Care Home Base (HCHB) and OASIS assessment tool.
• Strong clinical judgment and critical thinking skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Ability to work independently and efficiently manage a flexible schedule.
• Excellent communication, organization, and time management skills.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points weekly caseload.
• Familiarity with Medicare and home health compliance standards.

Schedule & Contract Details:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM to 5:00 PM.
• Typically 35 patient visits per day, with 2532 points per week.
• Mileage reimbursement provided.
