A skilled Home Health Registered Nurse (RN) opportunity is available to provide compassionate, high-quality nursing care to patients in their homes. This role focuses on delivering patient-centered care through comprehensive assessments, individualized care planning, and coordination with interdisciplinary healthcare teams. Key Responsibilities:

• Perform detailed OASIS assessments and develop customized care plans.
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and promptly notify the care team and physicians.
• Educate patients and families regarding disease management, treatments, medications, and preventive strategies.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and aides to ensure coordinated care delivery.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with all regulatory and agency standards for care and documentation.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent experience in home health nursing, preferably with field experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical judgment and critical thinking skills.
• Valid drivers license, reliable transportation, and auto insurance.
• Ability to independently manage a flexible, field-based schedule.
• Excellent organizational, communication, and time management abilities.

Preferred:

• BSN degree.
• Experience managing 2532 patient care points weekly.
• Knowledge of Medicare regulations and home health compliance.

Location Details: This role is field-based, covering patient homes within the Pine Apple, AL area. Daily travel within the assigned territory is required; mileage reimbursement is provided to support your mobility.

Schedule and Contract:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM to 5:00 PM.
• Caseload expectation of 2532 points per week (approximately 35 patient visits daily).
