If you are an experienced and compassionate Registered Nurse with a passion for home health, this is an excellent opportunity to make a meaningful impact directly at patients homes. Youll play a critical role in delivering patient-centered skilled nursing while collaborating with an interdisciplinary team to achieve the best possible outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Typical caseload: 2532 points per week (35 patient visits daily)
Key Qualifications:
- Active RN license in the state of practice, in good standing
- Minimum 1 year of recent home health experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) required
- OASIS assessment expertise
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently and manage a flexible, field-based schedule
- Exceptional communication, time management, and organizational skills
Preferred:
- BSN degree
- Previous experience managing a 2532 point caseload
- Understanding of Medicare and regulatory compliance
Primary Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursing interventions: wound care, medication management, IV therapy, and chronic disease management
- Monitor progress and coordinate timely communication with the healthcare team
- Educate patients and families about diagnoses, medications, care plans, and preventive health
- Meticulously document care activities and patient status using HCHB
- Collaborate with therapists, social workers, and aides for holistic patient support
- Participate in conferences, meetings, and ongoing education as required
