Job Description
Take on a vital role providing compassionate care as a Home Health Registered Nurse. This contract position offers the chance to make a real difference by delivering skilled nursing services directly in patient homes, helping them maintain independence and achieve their health goals.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care, such as wound care, medication management, IV therapy, and chronic disease monitoring
- Track patient progress, identify any changes in condition, and communicate promptly with physicians and the interdisciplinary team
- Educate patients and families on disease processes, medication, and preventive health measures
- Ensure timely and accurate documentation using Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and home health aides to coordinate care
- Participate in team meetings, case conferences, and ongoing education
- Adhere to all state, federal, and agency regulations for home health care
Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong clinical judgment, communication, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently while managing a flexible, field-based schedule
- BSN and previous experience managing 2532 points per week preferred
- Familiarity with Medicare and home health compliance standards is a plus
