Deliver skilled, patient-centered nursing care directly in the comfort of patients homes as a Home Health RN. This full-time, 13-week contract position offers the opportunity to impact lives while leveraging strong clinical and assessment skills in a dynamic, field-based environment.

Schedule/Hours:

• Monday to Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete comprehensive OASIS assessments and create tailored care plans.
• Provide direct skilled nursing interventions such as wound care, IV therapy, medication administration, and disease management.
• Monitor patient status, recognize changes, and communicate effectively with physicians and care teams.
• Educate patients and families about health conditions, treatment protocols, medications, and preventive strategies.
• Document all care and communications accurately in Home Care Home Base (HCHB).
• Collaborate closely with other cliniciansPT, OT, social workers, and home health aidespromoting coordinated, integrated care delivery.
• Attend case conferences and staff meetings. Participate in ongoing education and regulatory compliance.

Qualifications:

• Active RN license in the state of practice, in good standing.
• Minimum 1 year of recent Home Health RN experience (field-based preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation, valid drivers license, and current auto insurance required.
• Self-motivated, organized, with excellent communication and time management abilities.

Preferred:

• BSN
• Experience managing a 2532 point weekly caseload
• Knowledge of Medicare and home health compliance
