An opportunity is available for an experienced and compassionate Registered Nurse skilled in home health care. This contract role involves providing high-quality, patient-focused nursing services directly in patients homes. The position requires clinical expertise, autonomy, and a dedication to patient and family education, making a real impact on the continuum of care.
Schedule & Details:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points weekly (approximately 35 patient visits per day)
- Field-based, providing care within assigned territories
- Utilizes Home Care Home Base (HCHB) for electronic documentation and OASIS assessment
- Daily travel with mileage reimbursement
Core Responsibilities:
- Complete comprehensive OASIS assessments, develop, and manage patient-centered care plans
- Deliver skilled nursing care: wound treatment, IV therapy, medication management, and chronic condition oversight
- Monitor patient progress and promptly communicate with the interdisciplinary care team and physicians
- Provide clear education to patients and families about treatments, medication regimens, and disease processes
- Maintain timely, accurate documentation within HCHB
- Collaborate with physical/occupational therapists, medical social workers, and home health aides to ensure continuity of care
- Attend case conferences, maintain educational competency, and comply with all regulatory and agency protocols
Required Qualifications:
- Active RN license in good standing
- Minimum one year of recent home health (field-based) RN experience
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments
- Strong assessment, organizational, and communication skills
- Reliable vehicle, valid drivers license, and proof of auto insurance
- Comfort working independently and managing a flexible field-based schedule
Preferred:
- BSN degree
- Experience handling a caseload of 2532 points per week
- Understanding of Medicare and home health compliance
