Job Description
Deliver compassionate, high-quality care as a Home Health Registered Nurse, providing skilled nursing services to patients in the comfort of their homes. This role is ideal for a clinically proficient and empathetic RN who thrives in a field-based, independent environment and values the direct impact on patient outcomes.
Schedule/Hours:
- Full-time, 13-week contract
- MondayFriday, 8:00 AM5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Field-based position with daily local travel (mileage reimbursement provided)
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized plans of care
- Deliver skilled nursing interventions, such as wound care, medication management, IV therapy, and chronic illness support
- Evaluate changes in patient status, communicate updates to the interdisciplinary team, and adjust care plans as needed
- Educate patients and their families on medical conditions, treatment adherence, and preventive health
- Document care accurately and promptly using Home Care Home Base (HCHB)
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate care
- Participate in staff meetings, case conferences, and continuing education for best practices
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health nursing experience (field visits strongly preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS documentation
- Strong clinical assessment and critical thinking abilities
- Excellent interpersonal, time management, and organizational skills
- Reliable transportation, valid drivers license, and current auto insurance
- BSN preferred; familiarity with Medicare/home health compliance is a plus
